Insurance and Health Plans Accepted

Please refer to the pages above for information about insurance and health plans accepted for hospital and clinic services. Insurance plans vary depending on the service being provided.

While every effort is made to ensure this list is current, please verify with your insurance before your visit.

Special Note (10-24-16):

• Eisenhower does not expect that there will be any contract changes between 2016 and 2017. We are currently working on renegotiating insurance agreements with Anthem Blue Cross, Blue Shield of California and Cigna. it is expected that Eisenhower will reach agreement with these insurance carriers prior to the expiration of the current agreements. The renegotiations will probably not be completed until December 2016 or January 2017 so a definitive answer cannot be provided until the contracts are finalized, however, it is anticipated that there will be no changes subject to successful renegotiations.

• Patients covered by Anthem Blue Cross have reported that they received a letter indicating that their PPO plan has been replaced with an EPO Plan. The difference between the current PPO plan and the future EPO plan relates to “out-of-network” benefits. There is no change to “in-network” benefits. Since Eisenhower Medical Center is in-network, there will be no change subject to the above comment about renegitiation..
