Advance Directive DNR for EMS
This directive is for the purpose of emergency medical responders (fire, ambulance, police) who may arrive to your home after dialing "911" or to the scene of an accident. This form specifies whether or not you have a preference to be resuscitated.
You may designate an adult relative, including a same-sex partner, or any other person you trust to speak for you when medical decisions must be made.
